  • Imię
  • Nazwisko
  • Data urodzenia

Warunki użycia:

  • Użytkownicy tego serwisu przyjmują do wiadomości, że prawa do jego zawartości są własnością osób fizycznych (posiadaczy certyfikatów) oraz Concept Systems Inc.
  • Aktywni Dive Professionals oraz Centra Nurkowe są autoryzowani do przeglądania tej zawartości, aby sprawdzić status właściciela certyfikatu w związku ze szkoleniem oraz nurkowaniami rekreacyjnymi. Zapotrzebowane dane można jedynie przeglądać na ekranie monitora.
  • Transfer danych osobistych do osób trzecich bez pisemnej zgody posiadacza certyfikatu jest zabroniony.
  • Tymczasowe karty są przeznaczone do osobistego wykorzystania wyłącznie przez posiadacza certyfikatu. W razie potrzeby karty te mogą być duplikowane przez posiadacza certyfikatu.
